How Medicare Drug Tiers Work

Every Part D and Medicare Advantage drug plan sorts covered medications into tiers. The lower the tier, the lower your cost. The structure below is standard nationwide — but each plan decides which drugs land on which tier, so the same drug can sit on different tiers from one plan to the next.

Preferred Generics

The lowest-cost, most common generics. Often the smallest copay on the plan.

Other generic drugs — still low cost, slightly higher than Tier 1.

Brand-name drugs the plan favors. Moderate cost — many well-known brands live here.
